What you will achieve
Create a reusable questionnaire, review its questions and publish a version ready to issue.
Publishing a template is not the same as sending a request. Existing issued requests keep their pinned version.
Before you start
Agree the purpose, intended respondents, required evidence and assessment owner. Check the template library first. A client-specific imported questionnaire may be restricted to that client's response and cannot automatically be reused for suppliers.
Compose the survey
- Open Assurance and select New survey, or open a suitable template.
- Use Clone template for a platform template rather than assuming you can edit the shared original.
- Enter a clear survey name and description. Explain what the answers will be used for.
- Select Add group to organise related questions in a reader-friendly order.
- Select Add question. Choose its group and answer type, write the question and help text, and decide whether an answer is required.
- For choice questions, add and order the options. Include a truthful route for uncertainty or non-applicability where the question requires one.
- Select Add to survey or Save question, then inspect it in the group.
- Review assessment settings where available. Ensure they match the evidence you expect, not just the desired score.
If using Mojo to generate content, review every question, answer option and assessment rule. AI generation is a draft-writing aid.
Save and publish
Use Save draft while the survey is being reviewed. Reopen it to verify saved groups, wording, required fields and order.
When approved, use Create template for a new published template or Publish new version for edits. Read the confirmation and inspect the resulting version status and publication date.
Check it worked
Open the template summary and review the published questions as a respondent would encounter them. Confirm it is eligible to issue for the intended use. A published label alone does not remove a client-specific usage restriction.
If something goes wrong

Human judgement matters
Ask only for evidence you need and have authority to receive. Avoid questions that force a misleading yes/no answer.
